Challenging Abdominal Aortic Aneurysm treated at Max Super Speciality Hospital, Dehradun

The people of Uttarakhand can avail of difficult surgeries closer to home at Max Hospital Dehradun, which is providing surgical treatments of the heart, lungs and blood vessels to the patients. The hospital has performed some of the toughest cardiac surgeries with the recent one being TEVAR to treat abdominal aortic aneurysm.

Dr Manish Meswani, Sr.Consultant- CTVS said, “We recently treated a patient through a challenging cardiac surgery. The patient had come for a different disease but was diagnosed with abdominal aortic aneurysm (AAA). The aorta is the largest blood vessel in the human body. It carries blood from the heart to the head and arms and down to the abdomen, legs, and pelvis. The walls of the aorta can swell or bulge out like a small balloon if they become weak. This is called an abdominal aortic aneurysm (AAA) when it happens in the part of the aorta that’s in abdomen.

Elaborating further on this he added, “The patient’s aorta dilated up to 8 cms which ideally should be only 3cms. It can be life threatening if it bursts or ruptures. Conventionally, in these types of cases an open abdominal surgery is performed which is highly invasive and takes longer recovery time. Whereas, we treated the patient with TEVAR –Trans EndoVascular Reconstruction by stenting. It is a less invasive form of surgery and involves using a graft to stabilize the weakened walls of your aorta. The surgery lasted for about 2 hours and the patient was out of the hospital in 48 hours.”

Another unique case of ‘flail chest’ was successfully treated at Max Department of Cardiac Sciences. Flail chest refers to a type of trauma injury in the chest. It happens when three or more ribs are broken, causing a segment of bone to detach from the chest wall.

“Most of rib fractures and chest wall bone injuries are managed conservatively; however, a patient with flail chest on ventilator and not coming off ventilator becomes indication for chest wall reconstruction. Till today, very few centers in India offer surgical correction for flail chest. A 44 year old lady who met with accidental injury in Haridwar was brought to Max Hospital, Dehradun. The patient could not be weaned off ventilator, thus we decided to proceed with chest wall reconstruction. It was a complex surgery where team of doctors from CTVS, Plastic & reconstructive Surgery and Orthopedic Unit worked together to give a new lease of life to patient and her family.”

Talking about the challenges he said, “Using bone drills and sharp instruments on chest wall where important structures like heart and lungs are to be carefully protected remains one of the biggest challenges in such cases.  Upon completion of surgery, patient who was on ventilator for last 15 days got extubated just within two days and was discharged a week after.”

Apart from these, surgeries like coronary Endarterectomy, Atrial Septal Defect, Gunshot Injury, and Triple Valve Replacement are also performed successfully by CTVS team spearheaded by Dr. Manish Meswani.
