Why India Must Make Clinical Research a Core Part of Medical Education

India stands at a pivotal moment in medical education as the National Medical Commission and the Indian Council of Medical Research move to embed clinical research, ethics, scientific writing and research methodology into training. Dr. Seema Pai, President, Indian Society for Clinical Research (ISCR), argues that clinical research should no longer be an optional academic exercise but a foundational component of a doctor’s education essential for translating everyday clinical observations into evidence-driven practice, strengthening patient safety, fostering innovation tailored to Indian needs, and building a resilient, self-reliant healthcare system.

India stands at a pivotal juncture in the evolution of its medical education system. The National Medical Commission’s collaboration with the Indian Council of Medical Research to integrate clinical research, ethics, scientific writing and research methodology into medical training marks a timely and significant step forward. If India is to build stronger healthcare systems, improve patient outcomes and drive meaningful innovation in medicine, clinical research can no longer remain an optional component of a doctor’s education – it must become foundational.

For many years, medical education in India has primarily focused on diagnosis and treatment, producing skilled and dedicated doctors. However, modern healthcare demands more. Doctors today must be equipped to evaluate scientific evidence, understand evolving therapies, analyse patient data and contribute to improving practices through research and observation.

Clinical Research is not separate from patient care. In many ways, it is what strengthens patient care.

Every medicine, vaccine, treatment protocol or medical device used in hospitals today is the result of years of rigorous scientific study and carefully conducted clinical research. Yet, despite India being home to one of the world’s largest and most diverse healthcare systems, exposure to research during medical education often remains limited to dissertations or isolated academic exercises. As a result, many young doctors may enter clinical practice with limited exposure to how evidence is generated, interpreted and applied. This represents a significant opportunity for India to strengthen its medical education framework and build a more evidence-driven healthcare ecosystem.

India also has the scale and diversity needed to emerge as a major hub for clinical research. The country brings together a large patient population, expanding healthcare infrastructure, growing digital health capabilities and a strong pool of medical talent. Government data indicates that India’s doctor population ratio has improved significantly over the years and now exceeds the World Health Organization benchmark. However, numbers alone are not enough. A larger medical workforce must also be equipped to think critically, generate evidence and continuously improve standards of care. The Indian medical system must also focus on the Indian patient diversity, local practices and development of guidelines for Indian patients based on the strong foundation of clinical research, which is only possible if there is training and adequate understanding of the processes involved in drug development.

Every day, clinicians in India encounter unique disease patterns, evolving treatment challenges and public health realities that require deeper scientific research. From the growing burden of cancer, non-communicable diseases and antimicrobial resistance to lifestyle disorders and gaps in healthcare access, India’s challenges demand solutions built on local evidence and understanding. Embedding research within medical training can help transform these everyday clinical observations into insights that improve treatment approaches, strengthen public health intervention and support better policy decisions.

At the same time, India’s investment in health research is still evolving in proportion to the scale of its healthcare ambitions. Strengthening research culture within medical institutions is therefore essential to building a more self-reliant and innovation-driven healthcare ecosystem.

Clinical research should not be viewed solely through the lens of industry-sponsored trials. Its scope is far broader and encompasses understanding which diseases affect the Indian populations, how they are different or similar to other populations, evaluating treatment outcomes, improving healthcare delivery systems, assessing public health interventions and driving the development of affordable and accessible innovations suited to India’s needs. It is the bridge between clinical experience and evidence-based improvement in healthcare.

A particularly encouraging aspect of the NMC and ICMR initiative is its emphasis on a broader, systems-level approach rather than a standalone academic addition. Integrating research methodology, ethics, scientific writing and structured research exposure early in both undergraduate and postgraduate medical training can help students gradually build confidence and competence. Over time, this can create a viable and meaningful career pathway for young professionals, while strengthening the pipeline of clinician-researchers and physician- scientists in India.

Equally important is the emphasis on ethics. Research without ethical responsibility can weaken patient trust and compromise healthcare systems. Teaching ethics alongside research methodology ensures that future doctors understand patient rights, informed consent, transparency and accountability from the beginning of their careers.

The operational aspects of a clinical trial and the focus on data quality, completeness and accuracy built on a strong foundation of overall quality framework and quality management systems cannot be emphasized enough.

There is also a need to ensure that translational research is included with digital and AI education in medicine as an integral part of the course and curriculum that is being developed.

India has an advantage at the national level. There is a potential for India to be seen as an internationally recognized leader in clinical research because of its scientific talent, patient diversity and scale of practice. As India continues strengthening its position in global clinical research, long-term leadership will depend not only on scale, but also on credibility, scientific rigor, transparency and patient-centricity.

The integration of clinical research within medical training will help develop clinicians who not only cure illnesses but also contribute to the development of knowledge for future generations.

Ultimately, clinical research will enable a healthcare system that continuously learns, evolves and delivers better patient outcomes.
